Concept analysis of transitional care

Objective To analyze the development of transitional care and its application,attributes cases,antecedents and consequences through literature review,so as to clarify the concept of transitional care in China from the perspective of nursing practice. Methods CNKI,Wanfang,PubMed,CINAHL and PsycINFO databases were searched systematically,and Rorgers evolutionary concept analysis was used as the theoretical framework for analysis and discussion. Results Transitional care had the following 5 attributes,namely patient-centered,transferring,continuity,coordination and educational support. The antecedents of transitional care included factors related to patients and their families,hospital-related factors and social factors. Quality of life,self-efficacy and length of stay in hospital were the consequences observed.The measurement indicators are mostly patients’ transition preparation,patient knowledge improvement,self-efficacy and treatment compliance. Conclusion Transitional care is a dynamic concept. With the development of the times,it is constantly updated and improved. In the future,the connotation of transitional care needs to be further expanded and analyzed in depth,which is conducive to the further development of transitional care.

Concept analysis of parental perceptions of child vulnerability

Objective To clarify the concept of parental perceptions of child vulnerability. Methods We searched studies on parental perceptions of child vulnerability from websites and databases,including CNKI,Wanfang,VIP,CBM,PubMed,CINAHL,Embase and Web of Science. We selected relevant papers from the inception of databases to March 2022 according to inclusion criteria. Rodgers evolutionary method of concept analysis was used. Results A total of 33 articles were retrieved. 4 attributes were identified on parental perceptions of child vulnerability,including interacting between the child and their parents,having experienced or being currently in a traumatic event,the child in a healthy or stable state at present,negative psychological reaction and behaviors of parents and the child. The prerequisites and factors include the child and their parents,hospital factors and social factors. Parental perceptions of child vulnerability easily affect the healthy growth of the child,reduce the ability of parents to take care of the child and waste medical resources. The measurement indicators are mostly the level of parental perceptions of child vulnerability,the level of parental protection and so on. Conclusion The concept attributes of parental perceptions of child vulnerability were identified by concept analysis method. Researchers can carry out more comprehensive and in-depth research,combining the attributes of parental perceptions of child vulnerability.

Effects of psychological contract intervention on subjective well-being promotion among new pediatric nurses

Objective To investigate the effects of intervention based on the theory of psychological contract on subjective well-being among new pediatric nurses. Methods A total of 112 newly recruited nurses from 2016 to 2017 were selected to carry out training on subjective well-being in a pediatric tertiary hospital in Shanghai. An one-year training program based on the theory of psychological contract was developed,including comprehensive pre-employment training,individualized training mechanism and incentives,timely and effective communication model,to provide multiple supports and incentives on life,work,feeling and profession. The General Well-Being Schedule and psychological contract scale were used to evaluate the effects of the intervention. Results After intervention,psychological contract fulfillment increased gradually,and the score of subjective well-being was higher than before(P<0.05). Conclusion Nurse subjective well-being promotion intervention based on psychological contract can enhance subjective well-being among new pediatric nurses,stabilize pediatric nursing team,but it needs to be further improved and to become a continuous program.

Application of motivational interview based on self-efficacy theory in caregivers of children with enterostomy

Objective To explore the effect of motivational interview intervention based on self-efficacy theory on caregivers of children with enterostomy.Methods According to literature research and previous studies,motivational interview program was formulated,which included designing motivational interview strategy,screening and training for interviewing nurses. Seventy-two caregivers of children with enterostomy were randomly divided into two groups by using digital random table method with 36 cases in the experimental group and 36 cases in the control group. Five motivational interviews were conducted in the experimental group on the basis of routine health education and follow-up during hospitalization and 1,2,3 months after discharge;while the control group only received routine health education and follow-up. Self-efficacy scores,anxiety levels and complications related to stoma were compared between two groups at 1,2,3 month after discharge.Results After 3 months of intervention,self-efficacy scores of caregivers in the experimental group were higher than the control group,and the anxiety level was lower than the control group(P<0.05). The incidence of peristomal dermatitis in the experimental group was significantly lower than the control group(P<0.05).Conclusion Motivational interview intervention can improve self-efficacy level of caregivers of children with enterostomy,reduce their anxiety level,and reduce the incidence of peristomal dermatitis in children with enterostomy.
